My Choice Trust Services and its Community Pooled Trust I is administered by Coordinated Care Alliance New York (CCANY), Inc., a New York not-for-profit corporation as Trustee, and Pathfinder Bank as the selected financial institution Co-Trustee. CCANY is a trusted not-for-profit organization that oversees two Coordinated Care Organizations (CCOs): LifePlan CCO, and Advanced Care Alliance NY (ACANY). Our trust services help people with disabilities qualify and maintain financial eligibility for government benefits while retaining funds for life-enhancing purchases. Beneficiaries do not need to be a member of our affiliated CCOs to be eligible for enrollment in our trusts.
